You should be able to obtain a disabled person placard and/or license plate tag for your car by presenting the DMV with a letter from your doctor. Mine (in NJ) simply stated that, ‘Ms XX has a breathing difficulty/condition that keeps her from being able to walk more than 50 feet without becoming extremely out of breath. Please issue her a disabled person ID and placard.” When the four-year ID is nearing expiration, you should receive a notice in the mail asking you to indicate if you are still disabled. There is a DMV form to be filled out. Obtain the form first before requesting the letter from your doctor, as it will need his/her signature. Happy parking. (Just kidding. I’d rather be healthier and able to walk across the lots.) You’ll need your doctor to provide medical records to prove you’re handicapped, they’re pretty picky about proof of disability.
How bad is your COPD. do you use O2..how old are you?.maybe you need to learn to breathe..once you are in the store how do you walk around? try to enroll in a pulmonary rehab program.they will teach you how to purse lip breathe..walking is actually good for your lungs.maybe you need a little extra O2 when walking..ask if you can have a walk study done.good luck to you.
YOU CAN. GO TO DMV AND GET AN APPLICATION, FILL IT OUT AND HAVE YOUR DR. SIGN IT AND MAIL IT BACK TO DMV .THERE IS NO CHARGE AND THEY RENEW AUTOMATICALLY
The goal of your doctor should be to make you well and as independent as possible.What I did was went to the motor vechile place picked up my form, explained to the doctor that his job is to make me as indepentant as possible and that doing so I would need him to sign this letter stating that I needed a tag, I explained to him by the time I reached the store I was unable to shop due to the length of the walk in the elements. Question do you use O2, do you have a pulmonalgist? Your pumo doc is the best one to ask but your MD should have your health in mind and want whats best for you.
In the state of texas.go to your local tax assessor/collector’s office and get the medical form (your tax assessor/collector is where you register your car)take it to your dr to fill out,return it to the tax assessor/collector’s office and pay the five dollars for the placard.
go to a different doctor and get a medical note, that is absolutely assinine, and call the state ombudsman or you r hospital or insurance case managher or social worker ,to help you get a handicapped parking sticker or plaque, your oxygen level drops when you are walking and deprived the rest of your body, heart lungs and brain from getting enough oxygen which is critical for survival let those idiots know that
All you have to do is go to the dmv site for your state and download the form for a placard and take it to your dr he fills it out right there and you take it to the tag place and it cost 5 bucks per placard. its very easy
